Okay, after reading about the plot online, either I have an overactive imagination, or the writers never came out and said what the "real" story is.

Anyways, here's my jumbled pseudo-interpretation of things.

Amber, Blondie, and Rocket aren't real. I'm also tempted to say that either Babydoll or Sweet Pea aren't real either. The only thing that makes me think Babydoll is real is the scene at the end with the "High Roller" and the psychiatrist and all that, where the psychiatrist explains all the trouble Babydoll caused. The same scene is the only thing that makes me think Sweet Pea is real; the psychiatrist saying Babydoll helped a patient escape. The way she says it makes it seem as if the patient didn't actually escape (maybe they died), but if that were the case, which one would it have been? And if the other girls were real, you'd think the doctor would have mentioned them in relaying the story to the "High Roller".

I'm really dubious about the existence of Sweet Pea because of the identity of the bus driver, and the scenes where she is on stage. When Babydoll arrives at the hospital, Sweet Pea is on stage, apparently reenacting the beginning of the movie (which is also shown to take place on a stage, if you are paying attention to the first 10 seconds of the film). Then there's the scene of her acting out Babydoll's lobotomy.

Rocket apparently represents Babydoll's younger sister.

Things that make me doubt the existence of Babydoll are mostly the same things that make me doubt Sweet Pea. In addition to the theater scenes, there's the fact that Babydoll does the "dirty" work in the fight scenes, while Sweet Pea turns her back, like Babydoll is a figment of Sweet Pea's imagination, or a "split personality" to handle all the really bad stuff. Or vice versa, with Sweet Pea as a figment of Babydoll for the purpose omg I am confusing myself now.

Or maybe Babydoll's sister represents Rocket (but Rocket is still an imaginary friend).

Also, Sweet Pea is the only other one besides Babydoll to have a sword, but that might just be random happenstance (Rocket has a tonto, Blondie has an axe).

Shit.

SUMMARY: Only Sweet Pea or Babydoll are real, out of the 5 girls, and just maybe both... but I'm not completely sold on that, because of the theater scenes and the bus driver.

I can't believe people saw this movie and only saw an action flick, or whatever crazy shit they were saying about glorifying violence against women or objectifying women or blah blah women blah blah... yeah, they had short skirts on and stuff. Clearly they should have been wearing nun's habits or business suits throughout the entire film.
